The table of contents entry will sometimes not format correctly after a save or a preview (e.g. the font will be too big, and there will be extra blank lines in the middle). One workaround is to change "{toc}" to "{toc}" just before saving (you need to be in advanced edit mode to see this, on the first line). It will usually revert back on the next edit, so it needs to be done again. Sometimes doing 2 previews in a row will restore correct behavior.
